Athletic Participation Requirements
SIGN-UP INSTRUCTIONS FOR 1ST SPORT OF THE SCHOOL YEAR
In order to participate in any of Longfellow's sports, including tryouts and practices, you must complete each of the following online steps and print and submit all required forms to the office prior to the start date of the sport. These procedures are required each school year. Please contact the school office if you do not have online access or need copies of the forms.
1. Athletic Handbook - Must be reviewed by Parent/Guardian and Athlete once per school year.
2. Athletic Participation Forms - Athletes must have the appropriate form on file each year.
a. Physical Form - initially required of each athlete. In addition to the athlete
and parent/guardian signatures, the form must be signed by a physician.
Physical Forms are valid for two years from the date of the physical.
- OR -
b. Alternate Year Form - use this form if the student has a valid physical
form from the previous school year. The Alternate Year Form requires parent/guardian and athlete signatures.
3. Concussion/Head Injury Agreement - This is a three-step process required of
the Parent/Guardian and Athlete once per school year.
a. Review a concussion video
b. Read the concussion information BEFORE attempting to complete concussion agreement
c. Print and Sign a concussion agreement
4. Contact, Acknowledgement and Fee Form - Provide any changes to student
contact information, acknowledge handbook review. Fees for sports will be collected AFTER the season is over.
Participation fee is $25 per sport ($15 for Gr. 6 Track).
SIGN-UP INSTRUCTIONS FOR 2nd or 3rd SPORT OF THE YEAR
If this is your athlete’s 2nd or 3rd sport, you must complete the Contact, Acknowledgement and Fee Form page and submit it to the office with the participation fee.
